Job description

Physical Therapy Aide

Job Duties include:

  1. Provides physical therapy services to residents and initiates referrals when appropriate and in accordance with Physical Therapy Department Procedures.

  2. Conduct screenings of residents at regular intervals to determine the need for intervention/treatment.

  3. Assess residents to obtain data necessary for treatment planning and implementation and conduct specialized evaluations as indicated.

  4. Interprets and communicates evaluation findings to residents, families and team members.

  5. Establishes individual resident schedules in conjunction with other team members

  6. Follow treatment plans developed by evaluating therapists identifying a target list, including short term and long-term goals and methods to achieve identified goals.

  7. Coordinates treatment plan with resident, family and other team members.

  8. Implements and conducts treatment as outlined in treatment plan.

  9. Monitors resident’s response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.

  10.  Develop appropriate home or community program to maintain and enhance the performance of the residents in their own environment.

  11. Documents results of resident’s assessments, treatment, follow-up and termination of services.

  12. Adheres to department and resident schedule and modifies as appropriate for the treatment regimen.

Job requirements

Must have graduated from a Physical Therapist Aide Program

Must have a Physical Therapist Aide Certification in Washington.
